Number 192 - apparenlty the pages got caught in the book when someone tried sliding the book in, pages 393/394 and 395/396 affected as pictured below, otherwise the book is new:
Lettered: 26 signed copies housed in a custom traycase and sold out at the publisher
We also have a PC copy of the lettered - "PC" copy which is identical in every way to a numbered or lettered copy but has the letters "PC" in the limitation line, the "PC" denotes "Publisher's Copy" and is one of a very few copies produced for use by the Publisher, usually for contributors.
