What we have here is the First State Proof referred to as "Blue Lines" with the original Cover Proof (does have crease).  The pages are bound in separate signatures.

And the original Cover art by Charles Lang, measures 9x12 with the image being 5.5 x 7.25.  There are notations on the edges as pictured. Appears to be watercolor on cardboard.
